Paper Registration Instructions

 

• Download and complete the below Registration Packet for the event you would like to register for

• Mail the completed forms as well as payment to:

                             Spirit Extreme                                 

                             1735 Kings Hwy                               

                             Swedesboro, NJ 08085

• Download the Roster Template and complete one for each Registered Team

• Once completed, email all Roster Templates to spiritextreme@gmail.com

• After full registration is received, you will be notified once your athletes legal guardians have been sent the Athlete Waiver via email

• If for any reason, a legal guardian prefers to complete a paper copy of the Athlete Waiver instead of using the online option, please use the below Athlete Waiver Form

• All Athlete Waivers must be either completed online or sent via email to Spirit Extreme at spiritextreme@gmail.com by the Friday prior to the event

• Please note, Athlete Waivers only need to be completed once per season
